Another great week of Grand Rounds! We started with optimizing our wet reads on MSK xrays then moved into a victorious CPC win in the diagnosis of giant cell artertitis with a patient presenting with CN VI palsy and diplopia. We then discussed the tips and tricks for hernia reduction before hearing a moving story of a harrowing AirCare transport for ECMO cannulation. We moved into R3 small groups covering obstetrics and labor from mother to newborn and finished off with our pediatrics team visiting to discuss emergent cardiac conditions.

Sound Waves for Shoulder Dislocations

Shoulder injury and dislocations are common reasons for patients to present to the emergency department (ED) for evaluation. As ED physicians we often must determine whether the shoulder is fractured, dislocated, or both. Most of the time this is done through the use of physical examination in addition to the use of a plain film radiograph of the shoulder.

 The use of ultrasound in the diagnosis and management of musculoskeletal injuries is becoming more common. While it currently does not supplant the use of radiographs, it can be a useful adjunct to the management of these patients.

This week we disccused interventions for the crashing asthma patient with Dr. Mand, ESRD and dialysis related complications with Dr. Scanlon, shoulder and elbow xrays with Drs. Crawford and Scanlon, and a case of intussusception in an adult patient with Drs. Hunt and Bryant. Dr. Gauger provided his case follow up on PJP pneumonia complicated by methemoglobinemia, and Dr. Li took us through the ins-and-outs of gastric lavage in a simulated case of calcium channel blocker overdose.
