This week we brought in the new Roaring 20s with an exciting lineup. Drs. Gauger, Humphries, Jarrell, and Owens led us through a simulated case of hypertensive emergency in a cocaine body packer. Dr. Gressick taught us about blunt carotid artery injuries, and Dr. Gottula discussed resuscitation of septic patients with comorbid CHF and ESRD. Dr. Ham took a detour from clinical topics to discuss how to go about finding your niche in medicine, and Dr. Doerning increased our financial literacy with his high-yield points. Finally, our colleagues from Cincinnati Children’s helped us learn the nuances of having difficult conversations with children and their families.

This week we continued our leadership curriculum with Dr. Pancioli’s lecture on the intersection of leadership and finance. This was followed by Dr. Klaszky with his R4 case follow up of a patient with cardiac tamponade, and then Drs. Baez and Continenza faced off for the most recent installment of our Great Debate series as they discussed chemical vs electrical cardioversion for atrial fibrillation. Finally, our colleagues from Cincinnati Children’s presented learning pearls about causes of and interventions for hypoxia in pediatrics emergency medicine.

The week started off with a discussion on research and ways to get involved during residency. We then got a primer on early pregnancy ultrasound and ectopic pregnancy from Dr. Stolz. Our yearly directives series covered personal finance/loans as well as the job search and contract negotiation. Dr. Stettler then gave us some pearls on how to manage the agitated patient. This was followed by Dr. Plash who discussed the removal of GI foreign bodies. We then ended with a visiting lecturer, Michigan Program Director Dr. Laura Hopson, who covered bedside teaching tips and tricks.

This week's Grand Rounds started out with another installment of our leadership curriculum led by Dr. Stettler, where we discussed how to identify and manage finance in leadership.  This was then followed by Dr. Makinen's small group session on thyroid diagnostics in the ED.  Drs. Gauger and Loftus then went head-to-head on a case of syncope, found to have a massive PE.  Dr. Miller then gave a great summary of current thoughts and future approaches to sepsis, followed by Dr. Murphy discussing post-ENT procedure bleeds in the ED.  Dr. Curry then finished off the conference with his "Mastering Minor Care" segment on epistaxis.

Acetaminophen Toxicity

  • 60,000 poisonings / year
  • ~500 deaths per year, evenly split therapeutic / suicide
  • Mechanism - acetaminophen to NAPQI by P450, overwhelming gluthathione stores
  • Alcohol, phenytoin, isoniazid all induce P450 and may increase NAPQI production. Chronic alcohol use will worsen toxicity when acute ingestion will competitively inhibit and can be preventative at producing the toxic metabolite
