SQuID Protocol for DKA: Impact on ED Length-of-Stay
With the advent of fast-acting subcutaneous insulin analogs over the past 20 years, multiple studies have demonstrated that subcutaneous insulin protocols for treatment for treatment of mild-to-moderate severity DKA are safe and cost-effective when compared to intravenous insulin infusion protocols in non-ICU settings. In the context of increasing ED and hospital crowding as well as limited ICU availability, this study aimed to evaluate the impact of a subcutaneous insulin (SQuID) protocol in the treatment of DKA on emergency department length-of-stay (EDLOS) and ICU admissions
Grand Rounds Recap 8.23.23
We start out this week with our Morbidity and Mortality conference with Dr. Stark where she dives into a variety of cases included dog bite related infections, critical care boarding, malaria, septic bursitis, lunate dislocations, and regional wall motion abnormalities. Next, Drs. Arnold and Wilson discuss workup and management of patients with acute liver failure in the emergency department. Dr. Moulds then followed where she discussed ventricular tachycardias including Torsades de Pointes during her Taming the SRU presentation. Finally, we wrapped up the day with Dr. Kein’s R4 Case Follow Up where she discussed a patient who presented three patient cases who deserve your conservativism.

This week had a wide variety of topics including: cardiac arrest, SALAD, crying infants, and history! The day started off with a journal club covering some new critical care articles on cardiac arrest lead by our R3s. Next, the one and only Dr. Carleton gave us some airway pearls. Dr. Hughes educated us on the importance of handoffs and ED crowding. We had visiting professor Dr. Schmitz give us an excellent lecture on the past and future of EM. Finally our PEM colleagues taught us about fussy afebrile infants
