We start out this week with our Morbidity and Mortality conference with Dr. Stark where she dives into a variety of cases included dog bite related infections, critical care boarding, malaria, septic bursitis, lunate dislocations, and regional wall motion abnormalities. Next, Drs. Arnold and Wilson discuss workup and management of patients with acute liver failure in the emergency department. Dr. Moulds then followed where she discussed ventricular tachycardias including Torsades de Pointes during her Taming the SRU presentation. Finally, we wrapped up the day with Dr. Kein’s R4 Case Follow Up where she discussed a patient who presented three patient cases who deserve your conservativism.

Droperidol for Agitation in the ED - No Danger to the Dangerous?

Droperidol is a versatile medication with a number of potential uses for patients in the Emergency Department. It is also a medication surrounded in some degree of mystique because of the decision by the FDA in 2001 to issue a black box warning for its use in response to reports of QT prolongation and torsades de pointes. Many at the time (and since) have argued that, despite these case reports, droperidol is a safe and effective medication that can be used for the treatment of agitation, nausea and vomiting, and migraine. We have previously covered much of this background in a previous blog post. In our most recent journal club, we discussed 3 articles that looked at the safety and efficacy of droperidol for treating acutely agitated patients. Dr. Ham led off this week’s Grand Rounds reviewing cases from the month of November for our monthly Morbidity and Mortality conference. Dr. Chuko then led us through a deep dive on the diagnosis and treatment of male genitourinary disorders and was followed by Dr. Frankenfeld who led our small group discussion on STEMI equivalents. A battle of minds ensued between Dr. Gawron and Dr. Betz of an interesting case of traumatic rhabdomyolysis. Finally, Dr. Crook reviewed all of the latest important changes to the southwest Ohio EMS protocol.

This week in grand rounds we discussed all types of critically ill patients, first covering the spectrums of hypothermia and shock. We then had a fascinating case follow up on a patient who developed torsades des pointes, and learned how to perform the HINTS exam and incorporate it into our practice. We then discussed pediatric osteomyelitis and it’s subtle presentation, and finally covered musculoskeletal ultrasound of the shoulder, knee, and ankle.

Baez and Goel took us through cases of coding Torsades and coding Pine Sol ingestions, followed by Dr. Skrobut who delicately closed the day with a discussion of testicular complaints in the ED.
