It was a stellar week here at UCEM Grand Rounds! We kicked off the day with a lecture from our colleague at CCHMC, Dr. Dean, about management of pediatric airways. We then transitioned to a review of landmark studies in EM, specifically with regards to targeted and systemic thrombolysis for MI, stroke, and PE. We continued our day by discussing evidence based medicine and the implementation of this in the prehospital setting with EMS faculty, Dr. Richards. Next, Drs. de Castro and Thompson faced off in an epic battle of CPC where we covered the diagnosis and management of euglycemic DKA. This was then followed by Dr. Vaughan’s R1 diagnostics and therapeutics lecture on pneumothoracies. Finally, we wrapped up the day with Dr. Gillespie’s R4 capstone lecture.

For Grand Rounds this week We took a deep dive into 2 cases of Pericardial Effusions with our R2 CPC with Dr. Grisoli and our R3 Taming the SRU lecture with Dr. Ferreri. One case ended up being a purulent pericardial effusion while the other was an LV freewall rupture. Dr. Sabedra Expanded our knowledge on how to better care for pregnant and breast feeding patients and how to be a support co-worker to our pregnant and breast feeding colleges. Lastly, Dr. Frankenfeld walked us through some of the data around when patients leave against medical advice, how to try to avoid this, and how to appropriately counsel our patients.

We kicked off the month of March with a jam-packed Morbidity and Mortality Conference with Dr. Meaghan Frederick highlighting some of our amazing cases from downstairs including features on high sensitivity troponin, stridor and complicated alcohol withdrawal. Next up, Dr. Katherine Connelly walked us through how to troubleshoot in-transport alarms and failures of cardiac assist devices. Lastly, Dr. Bill Hinckley shared pearls of wisdom from his years at Air Care.

This week there were a number of underlying themes between pacemakers, MIs and leadership as well as some other high yield topics sprinkled in. Dr. Broadstock kicked us off with an educational lecture on pacemakers. We combined with our neurology colleagues to discuss how to best take care of basic neuro complaints in the ED in the time of COVID. R3s led an awesome small group on high sensitivity troponin. Dr. Banning shared the lessons she has learned through her own mistakes and failures. A truly heroic resuscitation for a patient in R heart failure by Dr. Skrobut in his Taming the SRU lecture. Lastly, learned about pediatric ophthalmology from Dr. Zamor.

This week’s grand rounds started off strong with Morbidity and Mortality led by Dr. Baez. She discussed a wide variety of topics including stress testing in the ED, precautions, hearing loss, aspiration, and tamponade. Dr. Randolph followed this up with an insightful discussion on high risk ED discharges. The Global Health Team then shared some of the fascinating cases they encountered overseas. Dr. Sabedra reflected on how much we learn from each other by giving a heartfelt talk on what she has learned from her fellow R4’s. We continued with Dr. Gawron reviewing the many cervical spine rules and how to properly apply them. To conclude, Drs. Skrobut and Roche went head to head in this weeks CPC. Who wins? Dr. LaFollette then discuss the oft-ignored complaint of hemorrhoids and peri-anal abscesses. We got snapshots from resident attendees of ACEP and Dr. Continenza discussed a case of postpartum hemorrhage resuscitation.
