It was another riveting week of Grand Rounds here in Cincinnati! Drs. Kate Connelly, Shawn Hassani, Allie Hunt and Eddie Irankunda kicked us off with a high stakes simulation case and discussion of aortic and vascular emergencies. Dr. Irankunda then reflected on his journey through residency via the filter of the 1990's classic TV show, ER, with many life lessons along the way. After a brief operations update with Dr. Fermann, we finished up with a case-based discussion of evaluation of vomiting in pediatric patients during our monthly combined conference with our PEM colleagues from Cincinnati Children’s Hospital.

In this week’s Grand Rounds, Dr. Polsinelli started us off with the diagnosis and management of aortoenteric fistula. We then discussed burnout and failure in our wellness curriculum in a small group setting, and heard from our colleagues in pharmacy about high-dose insulin therapy for calcium channel and beta blocker overdose. Dr. Roche led us in our Mastering Minor Care curriculum with a discussion of all things arthrocentesis, and we closed with a great combined PEM/EM simulation session and oral boards practice.

It's a frosty Easter morning and the ED is "q!&%t," all except for the 2 patient's turned over to you by the night ranger.  You greet the first patient, a 75 yo M complaining of flank pain - probably a kidney stone you think to yourself as you walk in to the room.  Walking into the room, you see the patient rolling around on the stretcher (as one would expect from those with a stone jammed in the UVJ), but something about his presentation strikes you as odd - a bit of diaphoresis, clammy pale skin.  It could just be pain, but the specter of a ruptured abdominal aortic aneurysm still looms large in your differential diagnosis.  You quickly exit the room, grab the ultrasound machine and head back in to take a look at his aorta...
