ACMC Safety Concern Form

Air Care and Mobile Care strive to provide the safest patient care possible. In an effort to ensure patient safety, reporting for incidents both clinical and operationally is paramount. This will allow for proper investigation and root cause analysis. This in turn will contribute to the development of educational processes to ensure errors are mitigated.

To submit a safety concern form:

  • Go to Staff Login from the main UCMC Air Care and Mobile Care webpage and enter password

  • Scroll down to the ‘Safety Concern Form’ section

  • Fill out the brief form:

Purpose: to increase awareness of safety hazards in the medical transport community

  • Voluntary process to share information regarding incidents/accidents where safety was compromised

    • CONCERN Bulletin

      • Submitted by air medical and critical care transport

      • Information regarding and accident or incident (fatalities, damage to aircraft sufficient to render it out of service, precautionary landings, etc.)

    • Hazard Awareness Reporting Page (HARP) reports

      • Submitted anonymously by any transport crew members at http://www.concern-network.org/index.php?id=HARP

      • Information regarding situations where no damage or injury occurs (not severe enough to warrant CONCERN Bulletin) but useful safety precautions or lessons have been learned

  • CONCERN Bulletins distributed via e-mail to CONCERN Network participants
